Polaris Health Directions has expanded its oncology distress management system to include Quality of Life modules to further support cancer treatment centers in their efforts to help minimize factors that can interfere with a patient’s treatment and overall quality of life.
With the system’s Quality of Life modules, cancer centers can identify the full breadth of patients’ behavioral, physical and personal struggles and then connect them with the appropriate programs and resources.
Cancer centers may also use the Quality of Life modules to assess a patient’s satisfaction with care, or whether a patient has followed up with recommended care through monitoring assessments and reporting.
Two of Polaris’s clients, Fox Chase Cancer Center and the Virtua Fox Chase Cancer Center Program, are already taking advantage of Polaris Oncology’s enhanced capabilities to alert care teams to any immediate needs a patient may have.
The Virtua Fox Chase Cancer Center Program, for example, is using Polaris to assess how patients are handling radiation therapy and other cancer treatments, as well as to screen for post-treatment quality of life issues.
In addition, the Polaris Oncology system sends triggers to the Virtua care team for patients in distress who need nutrition, social work counseling, complimentary therapies, or physical therapy, among other services.
Similarly, Fox Chase is using Polaris Oncology to send automatic triggers to their multidisciplinary team to alert them to patient’s individual needs, including rehabilitation, genetics counseling, tobacco cessation and spiritual counseling. The team is also able to request reports from the system to identify patients who are interested in or need a particular service, and then monitor who was contacted and who received the specialized care.
