Heart failure is responsible for more than one million annual hospitalizations, and more than half of these patients are readmitted within six months. To bridge the gap between hospitalization and the road to recovery, Southfield-based Quality Home Health Care Services of Michigan today announced the launch of its Comprehensive Cardiac Home Care Program, designed to incorporate medical care, technology, exercise and education to improve the health of heart failure patients in the comfort of their own homes.

“As February is American Heart Month, we feel that this is the ideal time to bring attention to the serious nature of heart health issues and provide Metro Detroiters with an outlet for in-home cardiac care, ” said Michael Gilliam, president and CEO of Quality Home Health Care Services of Michigan.

Cathy Nelson, a registered nurse with more than 30 years of cardiac care experience, will oversee the new program. Under her direction is a staff of cardiac and psychiatric nurses; physical, occupational and speech therapists; lymphedema specialists; medical social workers; home health aides; dietitians; and an exercise physiologist, all of whom will work with the patient’s cardiologist to execute a specialized in-home plan of care that aims to minimize hospitalizations and readmissions. A pharmacist also is available through the program and will visit patients in the home to review medications.

The QHHC Comprehensive Cardiac Home Care Program also leverages technological advancements to enhance its effectiveness. Portable EKG machines allow QHHC staff to transmit real time data to the patient’s cardiologist, granting the doctor the ability to monitor ongoing progress. The performing clinician also is able to correspond with the cardiologist through instant messaging via a secure website. Results also are accessible by fax, E: or through electronic medical records systems.

In addition, the program’s exercise physiologist has developed a regimen of chair and other exercises that are effective in assisting cardiac patients with strengthening heart function, decreasing fall risks and improving range of motion. A leave-behind DVD also has been developed for patients, intended to encourage ongoing participation.

An educational element also is incorporated into the overall plan of care, which focuses on teaching patients about the signs of potential congestive heart failure to prevent further hospitalization; methods to record and monitor vital signs for irregularities; guidelines for nutrition; and understanding medications, side effects and interactions.
